This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application. Learn more. DUTIES INCLUDE: Advocate for the Care in the Community model with VA Salt Lake City Health Care System, (SLCHCS), leadership, the medical community, and academic affiliates. Maintain accountability for all equipment and financial resources utilized in Care in the Community programs. Manage, support and ensure the continuation of the Care in the Community programs. Participate in the development of the annual business 4Cast plan submitted to facility leadership. Develop and implement measures to ensure efficiency and effectiveness in the designated areas of responsibility. Provide leadership to the VASLCHCS Care in the Community, implement reduction of Care in the Community within VASLCHCS and across VISN 19. Create and maintain strategic partnerships with VHASLCHCS program directors and service chiefs. Develop outcome measures to enable the Director and facility leadership to monitor Care in the Community program-level and individual performance and outcome metrics. Support clinical training and continuing professional development throughout the Care in the Community service line. Promote a robust education program to educate multidisciplinary health care trainees on the Care in the Community model of care. VA offers a comprehensive total rewards package.
