The Community Care Service Nurse Manager is accountable for ensuring the highest quality of care for patients in contract Community Nursing Homes (CNH), skilled nursing facilities under the Community Care Network (CCN) and for Veterans receiving care through Purchased Long Term Services and Supports programs. This position reports directly to the Chief, Community Care Service.
