The Home Based Primary Care Program (HBPC) provides primary care services to Veterans in their home in the Topeka, KS and surrounding areas. The Staff RN will function as the Care Manager, linking the patient and the caregiver with various health services that are offered by the multi-disciplinary team and the health care system. The professional nurse utilizes knowledge of nursing practice to provide care to patients in their homes. Basic Requirements: English Language Proficiency.
