This position is located in the Primary Care service of the Stratton VA Medical Center. You will function as a integral member of the Patient-Aligned Care Team and will be expected to work closely with team members, including nursing and administrative staff, in the coordination of patient care. You will participate in daily huddles with team members and in primary care systems redesign projects. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
