SUICIDE PREVENTION CASE MANAGER SOCIAL WORKER
The principle role of the Suicide Prevention Case Manager (SPCM) is to coordinate and monitor services for Veterans designated as high risk for suicide and act as the transitional coordinator of care for these patients. The determination of risk level is based on clinical evaluation and assessment of a Veteran by an appropriate behavioral health care provider. The incumbents are responsible for coordinating the care of high risk patients throughout the medical center, CBOC's and community. Collaborate with inpatient treatment providers/practitioners to support appropriate discharge planning following recommendations for treatment of patients at high risk for suicide.
